Heart attack...

Myocardial Infarction[MI] or in plain english heart attack kills people....in just a 6 hours shift yesterday i came across 4 such situations..all of them referred from private physicians...unfortunately these poor people keep roaming from one practitioner to another with no treatment and finally end up here losing that precious 3 hour window period where any medical intervention perhaps will save his life.....

am more worried about these atypical presentations[unusual symptoms] of MI...like this one guy yesterday who came with an upper abdominal pain and when i did a fresh Ecg[high suspicion coz of age and associated diabetes] turned out to be angina.....this patient could have been easily neglected as a case of gastritis and put on wrong drugs...

Memory is one thing which everyone cares about and would do just anything to have it..i have seen many people complaining abt it that they just dont remember anything and everything...but how many of those really work for it[like any sports or art] sincerely??...only a hand full!!...
When i was in 2nd year mbbs my main problem was to remember things as it is from the text[to score marks!!] and many times used to get frustrated for not able to do so...
One day by chance i got hold of this wonder book written by H.Lorayne.In this book he explains many good practical techniques[like link system,peg,mnemonics and many more] with demonstrations...i have tried using these techniques and have found them to be quite useful!!...

Marfan syndrome

Marfan syndrome (or Marfan's syndrome) is a genetic disorder of the connective tissue affecting about 0.02% of the population....People with Marfan's are typically tall, with long limbs and long thin fingers.

Three women turned up at hospitals over a 5-day period, all with the same symptoms.

He did (as did all of the other victims) have what was determined to be a puncture wound, on his right buttock. Investigators discovered that the flight he was on had originated in India .

The Civilian Aeronautics Board (CAB) ordered an immediate inspection of the toilets of all flights from India and discovered the Two-Striped Telamonia (Telamonia dimidiata) spider's nests on 4 different planes!
It is now believed that these spiders can be anywhere in the country.

So please, before you use a public toilet, lift the seat to check for spiders. It can save your life!

Cushings syndrome

yesterday we came across an interesting case[and a rare one too!] where the patients only chief complaint was excessive weight gain[abt 30-40kgs!!] in just 2 yrs...on investigation it was found out that he is having Cushing"s syndrome-an endocrine disorder caused by high levels of cortisol in the blood from a variety of causes[Wikipedia].....it was the first time we have come across this case and its usually seen in people taking excessive steroids[usually for relieving pain,asthma or after a transplantation]...luckily he hadn't developed any complications till now[such as the ones shown in the pic above] and hope that he gets well soon..:)

have seen many come backs in my life...but this is of a different kind...today my elder bro developed the similar kind of rash on his body as i had developed on my 3rd day of illness [last week]...and also some mild joint pain and am damn sure this time its not any viral infection but Chikungunya...actually there is no cure for it except some safe NSAIDS[non steroidal anti-inflammatery drugs] and some diet modifiactions[and of course mosquito repellents]...hes is doing just that and will be all right pretty soon...

Sincere apologize to my fellow bloggers for not commenting and replying to their posts these days...my pc also has some viral problem since everytime i go to any blog[or website] it gets restarted!..have to check it out....dont worry am reading all ur posts...and will be replying pretty soon!..:)

another short trip to Mangalore coming up [of one day] and really looking forward to it..postings till today were quite good where we saw many good cases....finally [and luckily for us:)] got to see a patient with Brocas Aphasia [see my title:D]...yeah there really is a term called Brocas which corresponds to area no 44 in the brain and is roughly situated above and front of our ears ..its anomaly results in a condition called Motor aphasia where the patient[as in our case] inspite of understanding what u say is not able to speak....actually this patient was hypertensive [SBP was 240!!..normal is 120-140] and had brain attack [similar to heart attack] due to this....
yesterday downloaded National Treasure 2 [fan of Nicolas cage!]..will watch it now and then start packing for the trip!!!!!:))....bye!!

TUNIS, Tunisia, April 27 (UPI) -- A Tunisian family alleges their daughter was raped during a telephone conversation with a man, a lawyer for the family said.

The 30-year-old man said he never touched the young woman. But he acknowledged he heard her scream while they were "totally into" an erotic telephone conversation -- and that she reported bleeding, Al Arabiya reported.

Maha al-Metebaa, a lawyer representing the family, told the Kuwaiti newspaper Al-Qabs the case needs careful investigation because of its unprecedented allegations. He said a medical examination had determined that the woman, 20, was no longer a virgin.

"The intercourse did take place with all its details but verbally only," he said. "The sexual act did not really happen because the physical proximity factor is not there, yet it happened because there is a direct physical impact – the loss of virginity."

medico-Legal definition of rape [atleast in India]-a man commits rape if he has sexual intercourse with a woman 1)against her will 2) with her consent by putting her in fear 3) false identity 4) with consent but at that time she was in a state of intoxication....

none of these things has happened in the above case and only god and he[the lawyer] knows how such a case can be valid for a legal procedure..!!!!!.......no wonder we are being exploited by the opposite sex!!!...:P

hmm......this is one thing we would have seen others doing [a thousand times] and we would have done it atleast once daily [if u are health conscious:D]..yeah u guessed it right.. its nose picking, also called "gold digging" or "rhinotillexis"...but unfortunately the part of the nose involved in this [lower part] and the upper lip are called danger area of the face coz any infection in these regions can directly spread to the brain [called cavernous sinus] through the veins..so be careful nextime when u are about to explore your gold mine!!!..:)

yeah finally relieved of this internals burden...can now relax a bit a for few days[skin postings :P]..paper was quite easy and not as usual lengthy!!!....have now planned to watch prison break or The Eye movie[looks scary in trailers!]..have medicine postings coming up so have to start preparing for it seriously[hope so:)]...yesterday while revising digestive system came across a condition called Anorexia nervosa -
"its a psychiatric diagnosis that describes an eating disorder by low body weight and body image distortion with an obsessive fear of gaining weight. Individuals with anorexia are known to commonly control body weight through the means of voluntary starvation, purging, vomiting, excessive exercise, or other weight control measures, such as diet pills.. this reminded of Christian bale who for his role in the movie Machinist had done the same thing [had lost more than 60 pounds!]...and usually those in professions where there is a particular social pressure to be thin (such as models) are much more likely to develop anorexia during the course of their career...dont u think they need a psychiatric evaluation???...
