am live

am still alive folks after 36 hours of nonstop duty[from monday morning till tuesday night with only 2 hours break in between]!!..yeah that's med where u get to do all kinds of work here from a ward boy's work to that of a consultant[read intern]...basically learning wise its a great curve but ultimately after doing so much work[coz of too many patients inflow] in the end u r so tired[and frustrated] that u loose interest in the cases and just feel like going home and take rest.
This was the first time in my mbbs life i felt that i shdnt have taken this course[in the morning after 20hrs nonstop duty].I mean even after working ur ass off[like a donkey] for nonstop 2 days u get hear complaints from the profs[even nurses here!].This makes me wonder here whether we are docs or dogs!!??[obey their every order and bear with their tantrums]

p.s-2 people actually tries to bribe while on duty...guess its not that they werent happy with our work[med as a whole],its just that they were concerned!!

EGO defence mechanism...

in Psychiatry theres something called Ego defense mechanism which are psychological strategies brought into play by various entities to cope with reality and to maintain self-image. Healthy persons normally use different defenses throughout life.Defense Mechanisms is to protect the mind/self/ego from anxiety, social sanctions or to provide a refuge from a situation with which one cannot currently cope.

Sublimation is one such kind[class 4 type] where there is Transformation of negative emotions or instincts into positive actions, behavior, or emotion...like a person loving nude/porn expresses it as a photographer or a painter[one famous painter here in India frequently does that]!!...

actually i loved this concept and have been really fascinated to do more research on it!!

went for duty work on monday morning...returned home today morning.....totally 24hrs duty....this will be our routine once a week[opd days] for atleast 4months from now on.....

this is in particularly true to those who get affected due a genetic cause[downs,albinos] like one patient who had this rare skin disorder[called xeroderma pigmentosa] coz of which her skin texture was completely lost and was discarded by her own father[due to nonacceptance] and hence come to our department for psychiatric evaluation.
p.s
- but the truth is its her father who needed this and unfortunately no one here[in govn hosp] has the time and patience to do that.

reality

after working in this department for 4 days i have realized one bitter truth...never like any subject theoretically for its practical aspects will be entirely different and in the end may disappoint you...to succeed here[in psych] u really need lots of patience[which i think i don't have] and a strong mind to cope with all the abnormal people u come across...
hope in the coming days at least i really enjoy working here..

still remember the first time[1] i gave injection when i was in 3rd yr...some things u learn only by doing !!

i take bath about 2 times a day[ much coaxing by my father..he thinks am a walking store house of germs since i go to hosp]...lets see till when i keep up with this habit...

i always carry 3 pens with me..all Parker...inkpen[which i use],dot and microtip....these parker pens have their own charm right?[or its just brand power]...

when i was in 2nd pu used to get up at 4 in the morning[of course to study]...tht was the first and last time i did that[not even after joining medical]

5 litres of sweat is the amount a person looses during exercise[its an entrance exam qn!] in this summer heat..so better watch out....keep urself hydrated guys!!

6 is the no of street dogs near my house...3 of those have died among those.....!!

there are totally 7 pgs in my med unit right now..3 out of them are from Nimhans....more pgs means less work for us interns:D

through out my middle and high school days i used to get up 8am on sundays...but now its either at 6[for jogging] or at 10...!!

ivy is the peg word for 9 and it also happens to my birth date.

i have atleast 10 watches[swatch,fastrack,titan octane etc] now in working condition ans still open to some more of them from anyone willing to present it..!!

actually i enjoyed taking up this tag...once again thank u reflections:)

a psychiatric case

history elicited from a patient in opd today-

"it started about 5 years back when i started hearing people conspire against me and when i got up[he was asleep then] looking out for him there was no one around or inside my house...since then am having this symptom quite frequently where i hear 2 people talk abt me.Even my wife is involved in this[he was damn sure about this] and i firmly believe that she has an affair with my employer .All 3 of them[wife,employer and his father in law] are together now and are conspiring to take away my property[17acres] and sometimes even control my thoughts..regarding this i have approached both police and lawyers and none of them are willing to help me"

this is a classical case of Schizophrenia where the persons reality gets affected....here in this case he had 3 of the 4 classical symptoms[called first rank symptoms]-Auditory hallucinations[hearing voices],Control thoughts and Delusion[false belief tht his wife is having an affair also called as othello syndrome].

@ home,stipend

finally at home..pavgada posting is over[and hence my rurals] and am so relieved and happy but at the same time sad coz it was such a nice place and i really enjoyed staying there...[not much work,free food and u can study a lot!!]
From saturday i have med postings which begins with psychiatry for the first 15 days...am really looking forward to this since i have kept an option open to psychiatry for my pg...working practically now in the department will surely expose me to its real pros and cons and will definitely help me in my decision making later.

p.s-My first stipend got released few days back.. am so happy...:)...this is my first earning in my life and the best part is that my whole mbbs expenditure will be given back to me in the form of this salary!!..that's the price u get for studying a govn college[of course u shd work ur ass off to get into it!!:)]

swine flu

swine flu has hit the people around the world really hard[atleast according to WHO] and is said be announced as a full blown pandemic[still in 5/6 alert phase-WHO with 1003 cases till now in 20 countries]..In simple laymen's language its a viral infection of the lungs leading to secondary bacterial infections[cause of death]...Luckily till now there hasn't been any case detected in India
And from this week we have been put for Airport duties.Our work there is just to screen the incoming international passengers for the flu with a questionnaire and isolate them in case found positive.Working there indeed makes u come across so many different kinds of people like one guy[around 50yr old] had to return to India coz he had a minor heart attack few days back[as told by his wife to her relative] and another guy who inspite of being well dressed[and with a 30k mobile] couldn't fill in the form coz he didn't know English.But the best were the air hostess whom i talked to while getting their filled in forms..each one looks so perfect that its really difficult to decide whos the best:D....

life in pavgada

finally am back from Pavgada[my present rural posting] after one full week duty...life was definitely good there but i missed my home there and now an happy to be back home[till sunday]..

its one of those few places which u instantly like and its a nice combination of work,enjoyment and studies...

we just manage odp work there[9 to 4 daily].Only 2 types of patients come here Tb or no Tb...here both DOTS treatment[for tb] ICTC[for Aids] and Mdt for Leprosy are provided for free of cost, hence u get to see quite a lot of such cases

most of the time we used to either study or watch tv....but sometimes u get bored doing this and for a change used to roam around the town in the evenings..
