flashback...

place-casualty
time-night[around 11pm]
people involved-ortho pgs,patients and ward boy

in ortho its a common thing to get fracture cases during night duties and in most of the cases to stabilize[and also to decrease pain] pops[plaster of paris] are put as a initial emergency medical aid and further managed depending on the patient[finance]/docs.

actual scenario-
a patient with obvious fracture comes to the emergency and one of my pg after initial routine[history,doing MLC and xrays] decides to put pop and tells[request-u cant order these self important jerks!] one of the ward boys to put the pop...after everything [putting cast] the ward order plainly demands money[pretty hefty] from the patient telling the dr himself has asked for it[pure exploitation].
After a few days some how this pg comes to know abt this[from the patient themselves] and so from then on in a heroic way[he was in 1st yr then] decides to put all the casts himself[in order to save their money]...satisfied/with pride[after putting some] at last walks into the duty room and narrates[to his senior pg] all this...senior pg then calmly replies "now tht u have personally gone to put pop ward boy would charge double the amount saying he has personally called u for the work"

p.s-
that was the last time he put the cast himself in his life....
those who think all govn docs r corrupt r wrong....

lady wolverine

just more days am out of this department...no more casts,night duties,pulling etc.
Never enjoyed ortho and wont be taking it as my pg.

watched Boondock saints-very stylishly filmed movie with good storyline...do watch it if u r a fan of such movies[like man on fire,usual suspects]

today had been my last OT[major postings] and was feeling all nostalgic...if i dont take a surgical field it will be last time ever i will wear a OT dress....

my personal[private] picture blog is now 165 posts old and it feels so good to see my yr old snaps....

p.s-
She-wolverine-snap taken by my co-intern after a k-wire fixation to a lady...

please...

"please sir just few prescriptions of these[/day]"...this is what i have been hearing from MRs[medical reps] in opds who visit regularly [whole week] and bombard the innocent docs[interns] sitting there with loads of new meds[trade names of same old drugs] and rarely sometimes with free samples...sometimes i find more MRs than the patients in the department wandering here and there with their bags and targeting/torturing every pg/intern they come across.More often it has so happened that its difficult to decide whether to talk to them or to the patients who keep pestering us with their year[like one patient who has a pain problem from 15 years but has never bothered to show it to any doc] old problems.
on the brighter side they r few advantages as well-free meds,pens[which i never use] and get to learn quite a few funky cool trade names[atleast it impresses the patient!] of the drugs.

p.s-
1-those of u who think am a sadist and experiment on patients like guinea pigs r mistaken...we write[most of the times] only those drugs which r safe and dont have any untoward rxns.
2-ortho is such a dull field-just xrays/pulling,pushing/mending bones....
3-had been to my native yest-@ seaside[west coast]

days ahead...

after 2 weeks of ortho i have-
put just 3 slabs[pops]
assisted in another 3-5
written about 50-80 painkillers[bread and butter of ortho]
in OTs have done practically nothing except for some pion and paper works...

in short-life in ortho sucks[compared to obg] with too many duties/ots....

p.s-
1-finally had been to 2012..quite a nice movie though 1st half was little boring..
2-MRs[medical representatives] come to every opd and give us a list of latest drugs in the market[coax us!] which we experiment on the patients...write the same drug for few of the patients and in case u get bored[or until u memorize it properly] jump to another one..lol..and its quite fun!!;)
3-studies have gone gone for a toss...hardly i study these days...:(...may be coz of lack of motivation plus too many duties...

the game begins now...

no more free days[atleast for the next few weeks] where i can just ideally in opds and while away my precious time...its time for some broken bones,nut.bolt,pops and muscle flexion[to pull,push etc!]

p.s-
from tomo actual ortho begins...had physiotherapy till now...
2 night duties in 4 days:( and moreover this rain has so much increased the incidence of rtas making our life miserable!![every 3rd accident case coming to casualty]
saw boondock saints movie...one cool action movie

in boys club

after ladies club its time to be in a boys club..i mean after obg its time for ortho postings...its been 2 days since it has started and till now[as expected] havent come across a single female[any age,height or weight!] in the department[not considering sisters and ayas]....

even the cases i come across are so monotonous...either fracture/rta[rd traffic accident] or some chronic pain case...and now itself i have realised this subj is not my cup of tea[as pg] and hoping to finish it as soon as possible and move on with my other postings.

p.s-

1-a thief when caught red handed and tried to escape fell from a roof top and unfortunately landed up in our department[me on duty] yesterday with 3 broken bones.!!

2-i really miss those cute babies[and babes] in obg!!:(

3-thanks Saranya for the award!:))

the calm before the storm

there is a saying in english "the calm before the storm" which means a peaceful and quiet period before a period of activity or trouble...thats exactly is the thing happening in my life right now...for past few weeks we didnt had any internals[other than clinics of respective postings] but now all of a sudden we have quite a few internals coming up back to back in just a short interval ...

30th of this month-obg full portions!!
31st of this month-surgery clinics
1st week of nov-surgery theory internals full portions!!
2nd week of nov-orthopedics internals full portions...

am so dead!!..wish me good luck[need lots of it!]

Vs time

planning is one thing i really believe in and right now am doing just that for the upcoming medicine internals...we[students like me] are now expected to study 350 pages of medicine within a short span of 6 days(hope so).I could have started med 2 weeks back itself when it was first announced but since had orthopedics postings didn't feel like doing so [its unfair to neglect our clinical postings subject].This is one problem we are facing from past 2 years.What happens is that during our clinical postings usually other subjects theory internals are kept and we are unable to decide whether to concentrate on our clinical postings or on the internals.Some serious brain storming needs to be done on this issue....my plans for today is to start with endocrinology and finish with infectious diseases by sunday [a really tough job!?]...hope i succeed in this task!

official!

yeah..its official ..our results were out 2 days back and am officially into the final year of mbbs[the final battle in the war:)].My results were not at all as expected(disappointed)!.After seeing my result i could have done 2 things 1) brood over that all day and night and ruin my mind!...2)apply one the dale carnegie"s principles[don't worry about those things which wont effect you after 5 years] and move on and guess what i did the latter!!!!...The thing which pissed me was that even after working hard for the exams i landed up in such a state..there is some thing seriously wrong with my approach to the exams or there is fault in the system itself...At the end of the day what matters the most is the our clinical knowledge and liking for the subject and all these marks don't matter much!!..Moving on to the final phase, our first clinical postings[orthopedics] will be over by Saturday and from next week for 2 weeks have skin postings and have almost decided to bunk it and prepare for the upcoming medicine internals!!!....

medical terms...

usually when studying we medical people come across many difficult terms which we have to remember and regurgitate it out in our exams!..here are few of those ultra complex terms which i have come across in the last 31/2 years of my medical training!!!

1) Pneumonoultramicroscopic silicovolcanoconiosis-a lung disease caused by silica

2) Arbeitsgemeinschaft für Osteosynthesefrage(AO)-study of internal fixation of bone fractures

3) dysdiadokokinesia-difficulty in performing rapid alternating movements usually seen in cerebellar damage in alcoholics

4) Hepaticocholangiocholecystenterostomies - a surgical creation of a connection between the gall bladder and a hepatic duct[liver] and between the intestine and the gall bladder[organ below the liver].

a mechanic!

" person who uses tools to repair things (generally machinery) or works to keep things operating properly" is the wikipedia definition of a mechanic which can very well be applied to a orthopedician except that he works on the bones instead of on the machines!..We had our 3rd round OT[operation theater] yesterday and were very much convinced of this fact.Typically a OT begins with the patient arriving to the room on time and is given preoperative medications.Here (in orthopedics) a single operation requires atleast a team of 3 surgeons where the other 2 except putting pressure and exhibiting their physical force on the patient don't do anything else.No wonder orthopedics fails to attract the opposite sex [so called weaker ones physically:), yet to find a female orthopedician] and really pity all those people who work here[:(].then after the OT we were made familiar with some instruments which had all kinds of nails and screws.

UB

today doing our rounds we came across a patient[a admitted one] who along with a thigh fracture had sustained some injury to his urinary bladder due to which he had lost his urination reflex!...this patient had not voided from last 3 days and hence when examined by the professor his urinary bladder it was at the level of the umbilicus[normally not palpable ]!..when the prof asked the pg why hasn't he still put the catheter for artificial voiding he just told that he has been waiting for the bladder to get bigger to which the prof replied "how much bigger should it get??".

3rd week

its our 3rd week in postings now and still haven't got that seriousness that a final year student should have got by now!...not that am wasting much time now ,but when i look back at the end of the day just don't feel satisfied with the days work, guess every one feels like that[hope so:)].Today we had ward rounds with our unit professor and it was quite good.In the end when we said him that we wanted to present a osteomylitis[bone infection] case he just told a PG to take the class and went away and we as usual weren't interested to attend a pg`s class so just bunked it and went to college campus[hope he doesn't screw us tomorrow]. Our 3rd phase results may be announced within a day or so and am keeping my fingers crossed!..:)

a patient"s plight

This is a true incident that happened in our OPD 2 days back

One of the patients on the night before coming to the OPD had sustained a shoulder dislocation by due to a trivial fall.Unable to bear the pain he had decided to go to a near by orthoclinic in Doddabellapur.There the doctor had demanded 2000rs for the treatment.Being poor he couldn't afford it but after much coaxing by the doctor he agreed to pay 300rs and was finally put a sling[for immobilizing it] for his fracture and sent away.Next day when the same patient came to the Ortho department here in Victoria Hospital he was shocked to see the same person sitting in the OPD chamber and coincidently it happened to be our unit assistant professor!!!!!....you must be wondering now how i came to know about this?...the patient himself narrated all this to my friend during his history taking for the internals!!...

How many bones do we have???

Ever wondered how many bones our body is made of?..sorry no price for guessing..its about 206!!...typically at birth its about 350 and as we grow, due to unison of the bones the number decreases attaining constant number of about 206 in adults!...

Here is the breakdown:

- 28 skull bones (8 cranial, 14 facial, and 6 ear bones); !!!!!!

- the horseshoe-shaped hyoid bone of the neck;

- 26 vertebrae-back bone! (7 cervical or neck, 12 thorax, 5 lumbar or loins, the sacrum which is five fused vertebrae, and the coccyx, our vestigial tail, which is four fused vertebrae);

- 24 ribs plus the sternum(breastbone); the shoulder girdle [2 clavicles(collar bone), the most frequently fractured bone in the body, and 2 scapulae];

- the pelvic girdle[hip] (2 fused bones);

- 30 bones in our arms and legs (a total of 120);

- There are also a few partial bones, ranging from 8-18 in number, which are related to joints

so next time when you see a person try counting them!...lol

first pill

today was one of those miserable days where you had to do your routine work when the world around you[ my other classmates] is busy enjoying and relaxing.Yeah,we had to give our first orthopedics internals today[this years first one] in the opd.The day typically started with us going to the opd on time[9am] and the professors as usual coming late[after 10].5 of us were told to take a case which unfortunately ended with the other 2 taking it.remaining 3 of us were given another case [a complicated? one] but unfortunately which due to patient"s incooperation couldn't complete it[SIMPU-students expert in making patients uncooperative??].Finally[thank god!] a pretty easy case was given ,which we presented to a assistant prof and got an idea regarding the internals in the coming days!
finally came home and watched MITHYA movie[produced by Arindam Chaudary-still guessing what made such a person enter the film industry!],it was good!

other part!

this is our 2nd week of our clinical postings and it started off quite well today in the wards.Took a wrist fracture case of a nurse[who is working here in the same hospital]. and who is been admitted in one of the special wards[yeah... government hospitals too have them but its not free].We were really surprised by the way she cooperated to us(usually medial staff people when admitted don't cooperate), and after finishing this case went to the rounds [for the 1st time] with our unit professor[who looks more like a zombie than a professor] and two of his PGs where we were shown some Xrays and some pretty bad RTA cases.Our theory classes were supposed to start today with a medicine class but fortunately:) that didn't happen.But the best part was got to meet some of my friends after quite a long time[ 3rd phase exams].
