obg,virus

hmm..finally our obg[obstetrics and gynaecology] postings[of 6 weeks] will be over today..it was quite a good posting where we learned many things and saw some really good cases...
we had internals on wednesday and my case was a lady[obvious] with a Rh -VE pregnancy[which means her blood grp is O-ve and her husbands is B+ve]...the problem in such pregnancies is that the fetus being Rh+ve[which acquires husbands blood grp-Mendelian principle] triggers a immune reaction in the mother which eventually results in its[fetus] rbc[red blood cell] destruction[similar to what happens in a mismatched blood transfusion] resulting in serious complications like jaundice in the baby...my patient had similar complication which unfortunately resulted in delivery of a dead baby:(...hope next time she conceives a healthy child..:))...
my pc is not working from a few a days due to some worm attck....feel like am in a jail....now at my aunts place and trying to blog here as much as i can:)...

everything commercialised!..


today i went to get my blood tested.....just wanted to do some basic tests like complete hemogram...these basic tests are really easy to do[since have done those myself at my uncles lab]..was surprised when i saw my bill..it was abt 400rs just for some 4-5 basic tests...
some rightly said medicine has really been commercialized these days and to be frank cant blame then also since they have to recover their investment!....

today was one of those few days where i studied hard for most the day and so its time for some relaxation now[ blogging!]...started with haematology today morning have finished about 60% now and hopefully will be able to complete it by night..while going through it came through a condition called Thalassemia where the blood hemoglobin synthesis is defective [quantitatively not qualitatively] and hence the patient will be suffering from anemia [decreased O2 carrying capacity of the blood].... in some of them this occurs as a trait [minor form] and in fact may confer to them some degree of protection in 2 ways 1) since in this condition life span of rbc is relatively decreased malaria parasites are unable to survive in the cell and so provides them survival advantage [to the patient]...this is common in some african regions...2) it decreases the incidence of heart attack in some[ not yet proved]...and in fact PETE SAMPRAS was a sufferer of this. [he disclosed this only after his retirement].

medical terms...

usually when studying we medical people come across many difficult terms which we have to remember and regurgitate it out in our exams!..here are few of those ultra complex terms which i have come across in the last 31/2 years of my medical training!!!

1) Pneumonoultramicroscopic silicovolcanoconiosis-a lung disease caused by silica


2) Arbeitsgemeinschaft für Osteosynthesefrage(AO)-study of internal fixation of bone fractures


3) dysdiadokokinesia-difficulty in performing rapid alternating movements usually seen in cerebellar damage in alcoholics


4) Hepaticocholangiocholecystenterostomies - a surgical creation of a connection between the gall bladder and a hepatic duct[liver] and between the intestine and the gall bladder[organ below the liver].

Physician"s nightmare


Normally a physician during his practice comes across many diseases diagnosing which becomes rather a complex due to multiple symptoms and causes.
DIC[disseminated intravascular coagulation] is one of them where on one side in the patients blood[intravascular] there is excessive clot formation and one the other side due to these clot formation most of the platelets[one required for clot formation] are used up resulting in excessive bleeding[due to resultant platelet decrease].This condition usually occurs during pregnancy and during road traffic accidents.When diagnosing and treating the patient for this condtion the physician is unable to decide whether to give procoagulants to stop bleeding(which further promotes clot formation) or to give anticoagulants(which in turn promotes bleeding) to stop the clot formation and thus it requires meticulous maneuvering of the clinician between Scylla of clot and the Charybdis of bleeding,rightly called as PHYSICIANS NIGHTMARE.
