1-a just born asphyxiated baby surviving after almost 10 minutes of nonstop cpr[by myself and a paediatrician]+adrenaline trail....we had almost decided to declare it dead to the party concerned...after successfull resuscitation it was shifted to nicu immediately.[but our concern was the amount of brain damage baby would have suffered due to the episode]
2-a lady delivering a "healthy" baby instead of urinating in bathroom...
3-an anaemic mother surviving the delivery and coming out of it as healthy as any mother....
4-me not delivering a single baby yestersay in 24 hours

god is indeed sending his much needed blessings upon the needy!;)

Preventive measures-

- Keep your immune system healthy. Eat plenty of healthy foods including fruits and vegetables, get enough sleep, and exercise at least 30 minutes a day most days of the week to keep your immune system in top form.

- Consider preventive therapy. If you test positive for latent TB infection, your doctor will likely advise you to take medications to reduce your risk of developing active TB. Vaccination with BCG isn't recommended for general use in the United States, because it isn't very effective in adults and it causes a false-positive result on a Mantoux skin test. But the vaccine is often given to infants in countries where TB is more common. Vaccination can prevent severe TB in children. Researchers are working on developing a more effective TB vaccine.
- Finish your entire course of medication. This is the most important step you can take to protect yourself and others from TB. When you stop treatment early or skip doses, TB bacteria have a chance to develop mutations that allow them to survive the most potent TB drugs. The resulting drug-resistant strains are much more deadly and difficult to treat.

This programme is in fact just a part of Universal Immunization Programme (UIP) under Child Survival and Safe Motherhood Programme (CSSM) which was setup in 1992.The UIP envisages achieving and sustaining universal immunization coverage in infants with three doses of DPT and OPV and one dose each of measles vaccine and BCG, and, in pregnant women, with two primary doses or one booster dose of TT.
Since we are posted for duty in a referral hospital now we are getting to see the real practical picture of this.Here weekly 2 days have been assigned for this vaccination thing during which vaccines are put to the child concerned for absolutely FREE of cost and this is being carried out in all the major phcs and district all over INDIA[almost 1000s] 365 days a year. Unfortunately many of u are still under the impression that a good health care facility is the one with a posh building with ultra modern facilities and good nice looking ambulances[and nurses!] parked outside but thats not the truth.The real thing is that which provides the basic of basic facilities to the absolute needy and that too free of cost and its all happening here right here in INDIA!.So next time before criticizing and pointing out fingers at the government plz think twice and appreciate the work done by it!!...

about the programme-
Its a de-centralized programme which implicates close association of the community and its leading lights and opinion leaders such as village Pradhans, Mahila Swasthya Sanghs, Primary School Teachers, etc. in formulation of the PHC based family welfare and health care plan. Under this approach, plans regarding family welfare services were to be formulated in consultation with the community at the grass root level and expected to lead to improvement in quality of services and client satisfaction matching with ground situation requirements
participation-
We have been assigned to this since the last 2 days and have been going house to house survey with paramedics.
Our[read paramedics:P] job is to just note down the family details[number,names etc] giving special importance to family planning and eligible couples[15-49yrs]....myself and my friend just accompany them to boost their moral(does it?!) and get the public co-operation[since most of these people think its just another some lame government survey and don't bother at all].
experience-
But the best part of such surveys is that u get to see the real family, their living style and its practical aspects like today we came across one where the children[2 of them] had developed chicken pox and their parents still hadn't got any medical opinion but instead had been visiting a temple hoping for the cure and also came across a house(medium sized) which was home to abt 7 couples[total 21 ] and they had a common kitchen,drawing room,store room!!...u really feel sick when u see this and in a sense little bit happy that u are in a better position in life.
p.s-government has sanctioned 180rs/day/worker for this and i still don't know where his money is going[paramedics who are in this project from last 1 days still havent got a rupee]

Pulse polio

Today[21st december] is the 1st phase of the two-phase pulse polio campaign[2nd phase is on feb1st] to be held all over India during which Oral polio drops will be administered to about two lakh children in the 0-5 age group.
About the Programme-
In pursuance of the World Health Assembly 1988, to eradicate polio by year 2000 AD. The Pulse Polio Immunization, in addition to routine polio immunization, was first started in the year 1995. Every child below 3 years of age was given two doses of oral polio drops with one month apart[September to feb-time of low transmission of this virus]. In order to accelerate pace of polio eradication, the target age was increased to cover children below 5 years of age from 1996.
The modified IPPI (Intensified Pulse Polio Immunization) strategy included vaccination of children through fixed booth approach on first day, followed by extensive house-to-house search of missed children for vaccination. During these years new strategies were introduced to cover endemic states and the results were good. At present mostly the transmission is going in Uttar Pradesh & Bihar.
our college participation-
Since our college is a Teritary Centre many nearby community centers are attached to it and as a part of our community medicine we were a part of 2005-2006 campaign...it was quite a good experience.The programme is conducted in 4 days,first day being the booth approach where 3-4 students were assigned to a booth[my place was in some temple!!] from morning to evening and on the other 3 days there will be extensive house to house search[each day 200-250 houses] for the children which are later recorded in a file and sent to the respective community centreheads...in the end we were paid some money and a certificate[10marks in psm are assighned to it]!!...
lets make this years campaign successfull by giving our full cooperation and participation:)

Smoking banned in India

The ban on lighting up rolled tobacco in public places came into effect from Thursday with hotels, restaurants, pubs, offices and even the international airports becoming out of bounds for smokers.
The ban will also cover even hookah bars and pubs as well as private offices and public places like bus stops.
Union Health Ministry issued a notification for the ban under the Cigarettes and Other Tobacco Products (Prohibition of Advertisement and Regulation of Trade and Commerce, Production, Supply and Distribution), Act 2003.
The ban has evoked mixed response from smokers and non-smokers with some calling it a "draconian" measure while others consider it a "god-send" gift that may help them in

medicine

our med internals got over today.this time we are posted in Victoria hospital and the unit we are in is quite good,every day classes are taken and to complicate things even the juniors[2nd year-the year when the clinical postings start] have been posted.Its quite fun teaching the juniors for the simple reason being u know more than them :P...
case given to me was quite complicated where the patient had breathlessness,cough and one episode of nose bleeding , and to complicate the diagnosis he had massive liver enlargement[about 20cm!!], finally some how came to co-relate all the findings the reason being Dengue!![our Pg really saved me]

Three women turned up at hospitals over a 5-day period, all with the same symptoms.
Fever, chills, and vomiting, followed by muscular collapse, paralysis and finally, death.There were no outward signs of trauma.
Autopsy results showed toxicity in the blood. These women did not know each other and seemed to have nothing in common. It was discovered, however, that they had all visited the same Restaurant (Olive Garden,Western Cape) within days of their deaths. The Health Department descended on the restaurant , shutting it down. The food, water, and air conditioning were all inspected and tested, to no avail.
The big break came when a waitress at the restaurant was rushed to the hospital with similar symptoms. She told doctors that she had been on vacation, and had only went to the restaurant to pick up her check.
She did not eat or drink while she was there, but had used the restroom
That is when one toxicologist, remembering an article he had read, drove out to the restaurant, went into the restroom and lifted the toilet seat Under the seat, out of normal view, was a small spider. The spider was captured and brought back to the lab, where it was determined to be the Two-Striped Telamonia (Telamonia dimidiata), so named because of its reddened flesh color.&nb sp; This spider's venom is extremely toxic, but can take several days to take effect. They live in cold, dark, damp climates, and toilet rims provide just the right atmosphere.Several days later a lawyer from Jacksonville showed up at a hospital emergency room. Before his death, he told the doctor, that he had been away on business, had taken a flight from Indonesia , changing planes in Singapore , before returning home He did NOT visit (Olive Garden), while there. He did (as did all of the other victims) have what was determined to be a puncture wound, on his right buttock. Investigators discovered that the flight he was on had originated in India .
The Civilian Aeronautics Board (CAB) ordered an immediate inspection of the toilets of all flights from India and discovered the Two-Striped Telamonia (Telamonia dimidiata) spider's nests on 4 different planes!
It is now believed that these spiders can be anywhere in the country.
So please, before you use a public toilet, lift the seat to check for spiders. It can save your life!

hmmm...
OBG postings started this week and its pretty interesting!...have seen some pretty good cases like DM in pregnancy,Anemia and also one IUGR case[intrauterine growth retardation]....very much to study:(...but overall quite interesting[and stimulating too!]!!! ...
