bipolar

this is our 2nd week in skin postings and we have started taking some good exam oriented cases [usually one skin case will be given as a short case in medicine]....as usual going through the cases admitted in the skin wards we came across a patient with a familiar face who had psoriasis...at first glance we were unable to recognize him but not giving much importance to this we decided to take his history [even though we knew about the diagnosis]..while doing so came to know that he had he was on lithium therapy and remembered that it was the same patient we had seen 2 years ago in the psychiatric ward (when we were posted there) for having a bipolar disorder (lithium therapy was one of the precipitating factors for his skin condition].
this really scared the crap out of me [u cant trust a person with a known psychiatric illness] but somehow managed to complete the case.When asked to come to the hospital skin opd with us [to present the case to the professor] he just refused[!] telling that an assistant professor had promised him to give him some medicine in the wards that day and he doesn't want to miss that [ this is a lie coz here in Victoria hospital no professor encourages medication in the wards]..this made us think that he still needs some of those psychiatric medications and out of frustration we asked our pg to convince the patient which she eventually did with the patient vowing to take any action against the pg if he happens to miss his so called medications..finally we presented the case to a pg and managed to learn some thing...
